The dedicated rehab staff—consisting of a physiatrist and several occupational, physical and speech therapists—makes use of current therapeutic methods and state-of-the-art equipment to identify each patient’s rehab needs. Our rehab team works diligently to assist each patient in regaining maximum functioning and facilitate a speedy, safe return to the community. From the first day of rehab, each patient—as well as family members—actively participates in setting attainable goals.
Our long-term care units are designed for individuals who can no longer be cared for at home or in an alternate living setting. We offer a warm, upbeat environment ideal for individuals who need assistance, medical supervision or extended care.
Our approach places a strong emphasis on restorative care, maximizing each resident’s potential functioning and mobility. Our long-term program is also designed to promote an attitude of independence. Residents are encouraged to choose their meal/activity preferences and perform tasks/activities self-sufficiently; this is crucial for emotional, social and physical well being. Additionally, residents are encouraged to bring personal possessions to create a homelike environment, such as rocking chairs, artwork, personal photos and other personal items.
Foothill Acres’ state-of-the-art facility is the home of a secured, 44-bed memory impaired unit, which is carefully designed to address the unique challenges of those suffering from Alzheimer’s Disease and other cognitive disorders. Our memory impaired unit provides a secure, calming setting to maintain and improve quality of life for all residents.
The environment is resident-oriented and family friendly. Each resident’s dignity is promoted, with a focus on health, safety and well being. Therapeutically designed activities encourage residents to retain and/or maximize intellectual and functional capabilities. Our specialized care program helps memory-impaired residents maximize their cognitive function, while our Snoezelen Room offers innovative sensory therapies, including audiovisual stimuli and aromatherapy.
Memory impaired individuals require an environment and care program designed to address their unique medical, social and recreational needs. Unit caregivers are sensitive and understanding of the condition and its impact on the resident and everyone involved in his/her life.
Respite Care
Oftentimes, caregivers require a short-term break from duties related to caring for a family member. Our respite program allows for peace of mind as patients benefit from nutritional meals, a comprehensive activity program and excellent quality care. Foothill Acres provides a comfortable, secure environment with 24-hour nursing care for as little as three days or longer, depending on the need.
Hospice Care
Foothill Acres offers Hospice Services for individuals who have life-limiting illnesses. This service focuses on the emotional, practical and spiritual concerns of the patient and the family. Care is directed toward relieving symptoms, maintaining or supporting function and promoting quality of life. Our team of professionals work together to make this time as comfortable as possible for hospice residents and their family members.
The Social Work Department works closely with the interdisciplinary team to ensure that resident and patient needs are being met on an ongoing basis. In the case of an individual who is being discharged from our rehab program, social workers meet with each patient to ensure that all services are in place at the time of transfer. Foothill Acres recognizes the important role that activities play in the health and well-being of our residents and patients. Our activity programs are designed to stimulate the mind and body, encourage social interaction and promote physical health and wellness.
Each resident has different needs, interests and capabilities. Our on-site Activity Director designs flexible schedules with a variety of programs each day, and encourages outdoor activity as much as possible.
