Here’s the latest update with the COVID outbreak at Foothill.
I’m happy to report all but one resident have recovered from the COVID Unit and are back in their regular rooms. We anticipate the last remaining resident will be cleared tomorrow.

What This Means

  • All units are returning to regular activities EXCEPT for the Sub Acute Unit which has one more week to go.
  • The Dining Room will be reopened tomorrow
  • We are planning to have a turkey carving ceremony in the Main Dining Room at the
    Thanksgiving Dinner for the residents.

Please keep in mind cases of COVID/Flu/RSV are still very high in our area. If you are not feeling well, please consider postponing your visit to the facility until you get better. We highly encourage everyone to follow CDC guidelines on getting vaccinated for COVID and Flu.

We had a new patient testing positive today from our subacute unit, the patient has been moved to our covid unit and place on precautions. On a good note I wanted to update you all that as of today we had 14 residents and all staff recovered and doing well. Since we are still experiencing this outbreak, we continue to advise to consider postponing visitation until the outbreak ends. If you do choose to visit, masks must be worn for the duration of the visit. As a reminder, we also offer rapid testing for anyone who wishes to be tested prior to visiting.

Good afternoon,

As we continue to monitor staff and residents every shift for signs and symptoms of COVID, we found 5 new cases (4 residents, 1 staff) We will continue to monitor all staff and residents during this outbreak. All COVID positive residents have been moved to the COVID wing on Cedar Unit. For those who have a loved one in the COVID unit, a separate email will be sent which will have instructions on how to schedule Facetime calls for your convenience if you choose not to visit in person.

Due to the early onset of Flu season, and considering we are in a COVID outbreak, we are strongly recommending not to visit if you are not feeling well. All visitors are required to check in at the kiosk at the reception area, wear a mask at all times while in the facility, and report back to facility should you get sick after visiting.

Please remember its vital to check out at the kiosk when exiting the facility.

Good afternoon,

As a result of our outbreak testing and symptom monitoring between yesterday and today we discovered 5 new resident cases and 1 staff positive for Covid. Staff and residents will continue to be screened every shift and next testing will be done on Wednesday and as needed for symptomatic residents and/or staff. We want to also inform you that in order to slow down the spread we had cancelled communal activities and dinning room however you may still visit, we ask that you please screen at the entrance, wear masks at all times even in the residents room and report if you have a positive test and recently visited one of our residents. We also like to remind you that we have available rapid tests at the entrance and we encourage you to take a test prior to visiting your loved ones. Please refrain from visiting if you are not feeling well.

We learned that yesterday one of our maple residents tested positive for covid-19, as a result residents were tested and 2 additional cases were noted as well (1 from maple and 1 from cedar unit) closed contacts have been notified and tested as well. All staff has been tested with no cases found. We will continue to monitor and test residents affected and will keep everyone informed of the results. I would also like to take the opportunity to remind everyone of when visiting to please keeping masks on at all times as well as refraining from visiting if you are feeling ill. Also, if you have visited one of our residents and later find out you test positive we ask that you notify the facility as soon as possible so we can monitor our residents accordingly.

As a followup to our COVID testing, all residents and staff from Magnolia Unit were tested today will no new cases. The employee is still out and recovering nicely.
 
We will be doing another round of testing next week and will be sharing results once known.
 
And speaking of COVID, be on the lookout for an email in the near future which will discuss the latest bivalent booster which we are planning on offering to all our residents and staff in the very near future. We can only get the Pfizer version at this time.

Today we tested all residents and staff on Magnolia for COVID and all results were negative. We will retest again and update families of those results once we have them. I am including the rest of last email to remind once again gently of the rules we still need to live by.

Earlier today an employee who works in Magnolia Unit tested positive for COVID. As per protocol, Magnolia residents and staff were tested today, and all results were negative. We will retest again on Monday.

Hello All,

Today a resident in our Sub-Acute unit tested positive for COVID. This was contact traced to a visitor who tested positive earlier in the week. The resident was moved to our COVID wing. As per protocol, all Sub Acute residents and staff are being tested for the next two weeks.

I haven’t heard back from anyone regarding the weekly conference calls so I will assume it is ok to leave it status quo with no standing calls scheduled. If there is anyone who wishes to start these up again, I am completely agreeable to it. Good evening 

Today 2 residents and 1 staff member from the Cedar Unit tested positive for COVID. Those residents were quarantined in our separate COVID Unit.  All staff that cared for this resident will be tested and followed for the next couple of weeks. All other residents on The Cedar Unit were tested and they were all negative at this time. Hello All,

I’d like to update everyone on where we are with COVID and review a few important policy items.

Thankfully Foothill Acres is COVID free, with the last resident recovering and going back to their regular room. All staff have fully recovered as well. The New York Times is reporting a daily average of just over 3000 new daily cases, which is half of what it was just a few months ago. CALI scores have dropped in multiple counties throughout NJ, but the COVID transmission levels in healthcare settings are high across the State. We are heading in the right direction, but still must remain vigilant in our fight against COVID. We respectfully ask anyone who isn’t feeling well to please consider not visiting until they feel better.

Another item Id like to discuss is the weekly conference call. I have been hosting the call every Tuesday at 3:00 PM, but after 4-5 weeks of no one joining the call, it was discontinued. Here is our latest COVID update.

 

Yesterday evening we learned one of our Sub Acute residents tested positive for COVID. The resident was transferred to the COVID wing. All staff and residents from Sub Acute Unit were tested with 2 staff testing positive. Additionally, one staff member from the Rehab Dept tested positive. We are suspending the dining room program for SUB ACUTE ONLY. Our long-term residents will continue to enjoy meals in the upstairs dining room.

Hello All,

 

Here is our latest COVID update.

 

Thankfully all residents remain COVID free. This is a direct result of our staff being diligent in Infection Control practices. I would also like to thank our families for their diligence in making sure not to visit if they aren’t feeling well.

I’d also like to remind families when celebrating a resident birthday with a large number of people, the facility would prefer to get advanced notice so we can plan accordingly. We offer families the option of throwing a party in the spacious Oak Room which can easily accommodate 30 or more people with plenty of room to spread out. Considering the CALI score is high, this also gives the facility the ability to control an exposure to a family member who may test positive shortly after the party.

Additionally, the facility strongly recommends limiting the amount of visitors to 4 people at any given time on the units. We are still offering free rapid tests to any and all visitors who wish to be tested.

As cases rise across the State, we are reminding all visitors that masks must be worn at all times while in the facility. We have been at the forefront of rolling out the 2nd booster for all residents who want it, and I’m happy to say the vast majority of our residents are fully vaccinated including the 2nd booster. The CALI score has jumped to Orange (HIGH) which means we all need to be extra vigilant in social distancing, and practice basic infection control. Last evening one of our NON direct care employees tested positive for COVID. Contact tracing revealed the employee most likely got the virus from her family. All residents that were exposed were tested today, and everyone was negative. They will be re-tested on Monday and again on Wednesday. We will be keeping a close eye on all residents and staff for signs and symptoms for the next 14 days.

 

We will also be keeping an eye on the New Jersey CALI score. It has been holding steady at YELLOW, moderate for the last several weeks. As case counts climb in across the country, it can have an impact on us as well.

Good afternoon,

Today I have some great news to share! Our Infection Control Physician determined that our last resident who tested positive was in fact proven to be a false positive, which means we were able to continue our clean day count and reached the required 28 days without infection. Armed with this, we applied and received the “End of Outbreak” designation from the local Health Department. I am happy to report that we resumed communal dining and communal activities.

On another note, we welcomed a new Unit Manager to the Maple Unit. Shawnta Jones has been a Nurse Supervisor at Foothill for a few years and transitioned to the Unit Manger position. Because of her previous experience, Shawnta is familiar with the residents on the unit, and we are very excited to have her in this position.

Infection rates statewide have dropped again, this time some counties are GREEN level.

Today I’d like to share some good news. Our last case of COVID was 10 days ago.What does this mean? Well, if all goes weas we hope and we get no new cases, I’d like to share the timeline on when we will be allowed to reinstate certain programs. Day 15 COVID free, certain group activities which don’t require the removal of the mask will commence.

Day 28 COVID free, we will apply for the “End of Outbreak” designation from the local Health Department. Day 29 COVID free, community dining will be reinstated. Infection rates statewide have dropped significantly. Here is the link to the latest CALI score. As you will see, Somerset Country dropped to ORANGE=moderate.

Covid cases in NJ appeared to have peaked and we see this with the CALI scores in almost all counties from “VERY HIGH” to “HIGH”. Currently in somerset county we remain at HIGH which is why is so critical that we continue to enforce infection control measures both in and out of Foothills.

We are continuing to move COVID positive residents into the long hall in Cedar which is the dedicated COVID hall, with dedicated staff to care for them.

 

The latest numbers are staggering, with over a million new cases reported we continue to strongly discourage families to visit.

We reopened the long hallway in Cedar and moved all COVID positive residents in that hallway, with dedicated staff to care for them to in an attempt halt the spread.

 

As this wave continues to overtake the country with nearly 489,000 new cases reported nationwide yesterday, (that is NOT a typo) we are strongly recommending families to carefully consider visiting during the next few days. Contact tracing has confirmed the recent outbreak sourced to visitation of individuals who were unaware they were contagious. We are re-launching our video calls program to allow virtual visitation. I hope everyone is enjoying the Holiday season.

 

As the next wave of COVID sweeps across the country, despite out best efforts we are still dealing with the outbreak. This week’s testing revealed one sub-acute resident and 3 residents in Maple Unit COVID positive. Additionally, we have 5 staff members testing positive as well. All positive residents were moved to private rooms in the designated COVID wing in Sub acute.

Although we have 97% of our residents vaccinated, the facility is requesting all families and friends to please not visit if you aren’t feeling well. The CALI score in the entire State is HIGH, which is a good indicator of the infections in the community. Additionally, regardless of vaccination status, if you are visiting you are required to always keep your mask on. Due to the outbreak, we have been forced to temporarily cancel community dining and activities. Foothill Friends & Family,

 

Happy Holidays!! We know that many of you have plans to visit Foothill Acres during the next few weeks or take your loved one out on pass to celebrate the holidays with family. Gratefully, there are no restrictions on these plans this year. However, we would be remiss if we didn’t review how to stay as safe as possible during the holiday season. Whether you are visiting the facility or having your loved one join you outside the facility, the #1 thing we ask is DO NOT VISIT if you are not feeling well and share that message to anyone who may be around your loved one at holiday gatherings. With the CALI scores high in the entire state, we are relying on everyone to do everything possible to protect all the residents at Foothill Acres and limit the possibilities of the spread of COVID.

 

We ask everyone who goes out on pass to please remember to practice infection prevention by using masks, good hand hygiene, limit the number of people your resident comes in contact with, and socially distance as much as possible. Each resident will be evaluated upon return to determine if any type of infection prevention protocols need to be implemented. This could include frequent testing, up to possible quarantine. If you have plans to take your loved one out of the facility and have not completed the Out on Pass form, please follow this link https://form.123formbuilder.com/5723781/form . We usually need at least 72 hours for Out on Pass requests especially if medications are needed. If there are any issues with a request, you will be contacted by someone from the nursing staff.

 

When visiting the facility, we ask that medical masks be used (we have them available at the front desk) and they remain on at all times while in the building, even when alone in a room with your resident. Please limit your movement around the facility and refrain from going into common areas such as dining rooms, activity areas, or lingering in the hallways. We also ask that you maintain socially distant space from other residents as much as possible.

 

We thank you for your continued vigilance and cooperation in being the first line of defense for the residents who live at Foothill Acres! We wish everyone a joyous and safe holiday season and let’s all look forward to 2022 with hopes that by next holiday season, we will not need any of these types of precautions or emails!!

One staff member (Nurse) who was working with the COVID residents on Saturday night tested positive. This staff member has not worked in the facility since she completed her shift late Saturday night.

Contact tracing was done and we successfully identified the source of the outbreak, which was from a family member who visited the facility several times in the days prior to the outbreak. The one really important lesson learned from this outbreak is while visitation is now permitted, I implore everyone to think twice before visiting the facility if you are not feeling well. Its easy to ignore symptoms and justify visiting but in today’s world I strongly feel the safety of the residents and staff should come before any individuals desire to visit. Additionally, we will be aggressively enforcing visitation rules on wearing appropriate PPE while on the facility grounds. If you are unsure as to what your personal responsibility is while visiting, please do not hesitate to reach out to us and we will be more than happy to answer all questions. There will be no exceptions to violations. We are hopeful this outbreak will be contained to the current case count.

As a followup to the earlier email, I’m happy to report that both Maple and Cedar tested negative for COVID. As things stand right now, we are holding at 5 cases in Sub Acute.

Good morning,

Late last night one of our short-term residents on the Sub Acute unit tested positive for COVID. This required us to test the whole Sub Acute unit which revealed another 4 residents testing positive. Residents who tested positive were moved into the COVID wing and are isolated from the rest of the facility. Additionally, we did PCR testing on all those residents that tested positive and are awaiting results of the PCR tests. We are currently in the process of testing the rest of the residents in the facility. Thankfully, the entire Magnolia Unit was completed with no one testing positive. As per the new guidelines, this will not stop visitation, but we implore you to stay home if you aren’t feeling well. Additionally, if you do decide to visit, we remind you that after checking in at the kiosk you must go directly to the unit you are visiting and when you are done to check out at the kiosk and exit the facility. No one should be walking thru the facility on a unit where you don’t have a loved one.

Magnolia= 0 residents

Maple=0 pending

Cedar=pending

We expect to complete testing of the Maple and Cedar units in a few hours and I will send a follow up email once I have the information.

Opening our doors and welcoming our families back home has brought tremendous joy to our hearts this week at Foothill Acres. Seeing loved ones reunite with our residents has been nothing short of amazing. However, we must continue to be vigilant as this pandemic is not yet over, and the safety of our residents is our number one priority. With that said, please remember that when you are here visiting your loved one, visitors are asked to limit their movement within the facility. Please do not walk around the hallways or go to different offices. Rather, go directly to the resident’s room or designated visitation area. We appreciate your cooperation in maintaining a safe environment.

It is with much happiness and excitement that we announce we have received new guidance from the Centers for Medicare & Medicaid Services (CMS) and we are now able to have Open Visitation!!!  Visitation is not completely back to pre-COVID standards, but a lot of the restrictions have been lifted.  Appointments will no longer be necessary and you will be able to “just stop by for a minute” if you are in the area!!   Please remember that COVID is still out there in the community and we ask for your assistance in protecting the residents and your loved ones by continuing to follow the Core Principles of COVID-19 Infection Prevention.  The following are those Core Principles:

Core Principles of COVID-19 Infection Prevention

  • Anyone who has tested positive for COVID-19, has symptoms of COVID-19, or currently meets the criteria for quarantine, should not visit or enter the facility.  
  • All visitors will be screened prior to their visit by using our Kiosk in the front lobby.  Anyone who does not pass the screen will not be allowed in the facility.
  • Visitors should follow good hand hygiene (hand sanitizer is available throughout the facility) and try to limit any surfaces they touch.
  • Facemask (covering mouth and nose) must be worn while in the facility and social distancing (6 feet or more) must be maintained when around other residents or healthcare personnel, regardless of vaccination status.
  • Visitors who are unable to adhere to these principles could have their visitation suspended or be asked to leave.

 

The New Guidance:

  • All visitation should be conducted in a manner that adheres to the Core Principles and does not increase the risk to other residents. 
  • All residents have the right to choose who and when they wish to visit and may, on occasion, deny or withdraw consent for a visit at any time.
  • Outdoor visitation is preferred when the resident and/or visitor are not fully vaccinated against COVID-19, weather permitting.
  • During indoor visitation, visitors are asked to limit their movement within the facility.  Please do not walk around the hallways or go to different offices.  Rather, go directly to the resident’s room or designated visitation area (if arranged in advance). 
  • If a resident’s roommate is unvaccinated or immunocompromised (regardless of vaccination status), visits should not be conducted in the resident’s room, if possible.
  • If Somerset County COVID-19 community level of transmission is substantial to high, all residents and visitors, regardless of vaccination status, should wear masks and physically distance, at all times. If level is low to moderate transmission, the safest practice is for residents and visitors to wear masks and physically distance themselves, particularly if either of them is at increased risk for severe disease or are unvaccinated.
  • If the resident and all their visitor(s) are fully vaccinated and the resident is not moderately or severely immunocompromised, they may choose not to wear face coverings or masks and to have physical contact.
  • Visitors should wear face coverings or masks when around other residents or healthcare personnel, regardless of vaccination status.
  • While not recommended, residents who are on transmission-based precautions (TBP) or quarantine can still receive visitors. In these cases, visits should occur in the resident’s room and the resident should wear a well-fitting facemask (if tolerated). Before visiting residents, who are on TBP or quarantine, visitors should be made aware of the potential risk of visiting and precautions necessary in order to visit the resident. Visitors also may be required to wear additional PPE.

Indoor Visitation During Outbreak:

An outbreak investigation is initiated when a new onset of COVID-19 occurs (i.e., a new COVID-19 case among residents or staff). When a new case of COVID-19 among residents or staff is identified, Foothill Acres will immediately begin outbreak testing in accordance with CMS QSO 20-38-NH REVISED and CDC guidelines. While it is safer for visitors not to enter the facility during an outbreak investigation, visitors can still be allowed in the facility.

  • Visitors will be made aware of the potential risk of visiting during an outbreak investigation and to adhere to the core principles of infection prevention.
  • If residents or their representative would like to have a visit during an outbreak investigation, they should wear masks during visits, regardless of vaccination status, and visits should ideally occur in the resident’s room.

Visitor Testing & Vaccination:

  • While not required, we encourage visitors to be tested prior to visitation
  • CMS as well as Foothill Acres strongly encourages all visitors to become vaccinated
  • Foothill Acres may ask about visitor’s vaccination status; however, visitors are not required to be tested or vaccinated (or show proof of such) as a condition of visitation.
  • If the visitor declines to disclose their vaccination status, the visitor should wear a mask at all times during the visit.

 

Our New Visitation Procedures:

Our standard visiting hours are 8 am to 8 pm.  We ask that you come to the front door and use the Kiosk to be screened.  It will print a visitor badge that must be worn while in the facility.  If you are vaccinated, please be ready to show your card to the front desk staff or, to expedite your check-in process, you can email a copy tofoothillacrescommunication@gmail.com.  In an effort to protect your loved one and the other residents, we do recommend unvaccinated visitors to receive a rapid test prior to your visit.  We have them available at the front desk.  They are quick and FREE!  Any visitor who is unvaccinated or prefers not to share their status, SHOULD wear their mask at all times while visiting.  Vaccinated visitors and vaccinated residents may remove their masks if they choose, while in their visitation area.  However, masks are required when in the common areas of the facility.  Unvaccinated residents (or their responsible party) may choose to have close contact with their visitors but assume the risks of possible exposure to COVID.

 

Once you have completed check-in, please proceed directly to your loved one’s room or other visitation area (if directed for infection prevention reasons).  There is no longer any time constraints or limits on the number of people visiting.  However, if you are coming with 4 or more people, you may be more comfortable visiting in an alternate location that can be reserved by emailing us in advance.  These areas are limited and available on a first come, first serve basis.  Visitors are not permitted in group activities or communal dining areas.  If you arrive at your loved one’s room and they are not there, please go to the nurses station and they will locate your resident and bring them to their room.  We have listed the main activity times and meal times below for reference.

 

When your visit is complete, please remember to wear your mask when leaving the room and go directly to the Kiosk in the lobby to check out.  A visitor restroom is available on the main floor, please see the receptionist for the key.

 

 

Out on Pass Visitation:

  • Residents who choose to leave the facility for outings with friends or family are strongly encouraged to follow all recommended infection prevention practices including wearing a face mask, physical distancing, and hand hygiene and to encourage those around them to do the same. 
  • Residents who go out on pass, will be screened upon return for signs or symptoms of COVID-19.  Unvaccinated residents are also eligible to leave the facility, however, depending on the COVID level of the community, may require testing or even quarantine upon return.

Other Information:

  • Prime Activity Times are 10:30–11:30 am,  2:30–4:00 pm, and 6:30–7:30 pm
  • Penthouse (upstairs) Dining Room times:  12:00–1:15pm and 5:00–6:15 pm
  • Calendly will be deactivated and the Essential Caregiver Program will be discontinued.
  • Foothill Acres remains COVID free at this time.

We know this will be an emotional time for everyone….us too!!   Unfortunately, the new guidance does not allow us to have large gatherings of visitors but it is a step in the right direction!!  We expect a few bumps in the road and ask everyone to be patient as we figure out this new aspect of visitation.  We understand some of you have never seen the inside of the facility beyond the lobby and the Oak Room, so please don’t hesitate to stop someone and ask for directions if you need it!  We look forward to welcoming you back into the facility in the next days and weeks!

We are very excited to report that we have welcomed several new employees to the Foothill Staff!!  What a pleasure it has been to see all of the new faces around the building and getting to know them as they learn their way.  We have an extensive orientation and training program for all new hires that includes a full day of mandatory inservices and learning about each department at Foothill Acres.  Our staff orientation day is scheduled for Tuesday, November 23rd.   Unfortunately, the only place to conduct this type of training while observing safe infection prevention practices is the Oak Room, which will conflict with indoor visitation.  We have closed visitation for that day, however we recognize that family and friends may want to visit during the week of Thanksgiving, so we have added afternoon hours for Wednesday, November 24th from 1:30-4:00 pm.  Please use the Calendly link below to schedule.

 

With the arrival of the colder weather, we are also ending outdoor visitation until the spring.  Anyone who is already on our schedule for an outdoor visit will automatically get moved to indoors at the same time.  You will receive a confirmation email from Calendly to confirm your new appointment.  Anyone who would prefer not to visit in the facility, may use the cancel button at the bottom of your confirmation email to cancel your appointment.  We understand not everyone is comfortable visiting inside the facility but it is just not ideal for the residents to be outside.  We do offer virtual visitation via video calls.  Please emailfoothillacrescommunication@gmail.com to schedule.

 

This year we are so happy to be able to celebrate the holidays with a little sense of normalcy inside the facility.  We will be having special activities scheduled on the actual day of each holiday including a visit from Santa and a party on Christmas Eve and a huge party on New Years Eve.  We have modified the visitation calendar for these days to allow the staff to focus on providing these celebrations for the residents.  We have added additional hours on the Wednesday of each holiday week to ensure that any resident who wishes to visit with their family, can be accommodated.  The dates are all listed below.

Good Afternoon,

 

As the weather gets cooler, everyone starts planning for the holiday season.  We understand spending time with family is a very important part of this time of year so we felt this would be a good time to review visitation guidelines.  At this time, there has been no changes to the directives from the New Jersey Department of Health regarding visitation nor have we received any updates for visiting during the holiday season. 

 

No changes will limit our ability to invite family and friends to join in our holiday celebrations within the facility, but it won’t limit our celebrating!!  We continue to have our dining rooms open and group activities daily. This week, we even had our first group cooking activity since before COVID!!  The residents had fun making pumpkin bars, but I think they had more fun eating them!! 

 

Some of our upcoming celebrations include a Halloween Party with costume contest, Thanksgiving dinner and Turkey Tailgate Party (eating deserts and enjoying football), Christmas Party, and a New Years Eve Party with confetti and bubbly!  If there is no change to the NJDOH visitation guidelines, we will host our 2nd Annual Holiday Shout-Out shows for Thanksgiving and Christmas, where family and friends can send us short video holiday greetings, music dedication, or even a short written note and photo for your loved one.  Our technology staff will put it all together and broadcast it to the entire facility via our private YouTube channel.  We will send out the link when it gets closer for everyone to tune in and join us!  You can start sending your greetings and shout outs to foothillacrescommunication@gmail.com.

 

Visitation:

Indoor and Outdoor social visits can be scheduled by going to www.calendly.com/foothillacres.  Keep in mind that as the weather gets colder, outdoor visits will be moved inside.  Please note that visitation may be limited on some days around the holidays due to the resident events taking place inside the facility. The key things to remember about these visits are:

  • Must be scheduled in advance no later than 4pm the previous day.
  • Visitors must be screened upon arrival prior to visiting with resident.
  • No food or beverages are allowed during the visit, however you can bring items but must leave with the staff during check in.
  • Each resident can have 2 visits a week, however if you check the website in the afternoon the day before and we still have open appointments, you are welcome to come more often.  Please understand if we have someone not able to schedule and we see a resident has had multiple visits during the week, we may ask to reschedule the appointment to allow another resident their visit.  (I really hope we have this problem because it means lots of residents are getting visitors!)
  • Visits are 25 minutes in length and up to 2 visitors, ages 2 and up
  • Residents not fully vaccinated must follow strict social distancing practices with visitors.
  • Masks must be worn at all times inside unless both resident and visitors are fully vaccinated AND there are no other visitors or residents in the same room.

 

Video Calling is a great way to stay connected!  We can do Facetime, Skype, Google Meet, and Google Duo.  These calls are done in-between group activities and meals so times may be limited.  Not sure how to do a video call, we can help!!  To schedule or get assistance email us at foothillacrescommunication@gmail.com .  The key things to remember about these visits are:

  • To allow time to schedule, please request video calls no later than 4:00 pm the previous day.
  • Video calls are 10 minutes in length.
  • Please include the following information when scheduling:  Resident Name, Type of device or platform you will be using ex, Facetime, Skype etc., Phone number of email address to contact you, Date and time you were hoping to receive call.
  • Calls are done 10am-7pm, and are not scheduled during group activities or meals.  Availability may also depend on staffing for the day.

 

Out On Pass Visitation is available so a resident can go out of the facility (off facility property) with family or friends for special occasions and family gatherings and functions.  There is an online form that must be completed to request an Out on Pass visit by clicking on this link,  https://form.123formbuilder.com/5723781/form. The key things to remember about these visits are:

  • Out on Pass visits must be requested at least 72 hours (3 days) in advance so medications can be reviewed and ordered from the pharmacy if needed.
  • A request form must be done each time a resident goes Out on Pass.
  • Residents who are not fully vaccinated may be required to complete a quarantine period when they return, depending on the Local CALI score at the time and/or the infection prevention practices observed when out of the facility.
  • Residents are encouraged to follow safe practices including masks, hand hygiene, and social distancing when Out on Pass in the community and in large gatherings.
  • Holiday Out on Pass requests may require additional time due to the large demand on the pharmacy, so please get them in early!  We will send out the dates from the pharmacy as soon as we get them.

 

Other Facility Information:

  • We had our first Pfizer vaccine booster clinic with an excellent turnout.  We will continue to offer boosters to the residents and staff as they become eligible for the injection.  We are monitoring the information regarding boosters for J&J and Moderna and will forward any information we receive about availability as soon as we get it from our pharmacy.
  • We currently have no positive COVID cases at Foothill Acres.

I know we were all hoping to be back to our pre-COVID holiday events this year.  Although we are not there yet, I am grateful that it is better than last year!  We are slowly seeing things get a bit better and maybe by the time the ball drops, we will all be able to greet each other in the hallways instead of at the front door!  We also want to thank all of you for your care and cooperation in protecting all who live and work at Foothill Acres when you visit.  We couldn’t do it without you!!

We are happy to report that the PCR test came back negative and visitation will resume starting Monday for everyone.  Those of you who already have an Indoor Visit scheduled for Monday are good to go and do not need to reschedule. Group activities, communal dining in the dining room, and The Salon will also resume starting tomorrow.

As you have heard in yesterday’s email, we had an employee test positive yesterday. We followed up with a rapid test and a PCR which both came back negative. We are very confident the first test result was a false positive which we will disregard pending the blessing of the local health department. We anticipate getting all clear by Monday, which means the earliest visitation would restart would be Tuesday.
 
We understand this is difficult having such disruptions to the visitation but with COVID, we can’t be too careful.

We were notified late this afternoon that a direct care staff member has tested positive.  They were wearing the appropriate PPE and were asymptomatic.  We are currently awaiting further testing to confirm the positive result but until then, all visitation, which includes Indoor Visitation in the Oak Room and Essential Caregivers must be suspended until further notice as we complete the first round of expanded facility wide testing.  We will forward more information as it becomes available to us.

Hello All:
 
I’m sure many of you are wondering about how Foothill Acres is doing after the storm last night. We had some flooding in the facility and had to move some residents from their rooms, but the greater concern was maintaining staffing levels since getting to the facility was a bit challenging.
 
I’m happy to report through heroics on the part of some very dedicated nurses and CNA’s, we were able to maintain the staffing levels to provide resident care. The “All Hands-on Deck” call went out and the response was heartwarming. Staff that lived in or near Hillsborough were able to report to the facility. We are anticipating no staffing issues whatsoever for the upcoming 3-11 shift.

Good evening,
 
Yesterday evening a new admission from the hospital tested positive for COVID. The resident was immediately sent back to the sending hospital, and the total time spent at Foothill was less than 2 hours. The resident was in an isolated room and staff that worked with her were using appropriate PPE. This is considered a non-facility acquired case and does not require 2 rounds of testing as its not considered an outbreak. As a precaution, all residents in the sub-acute unit were rapid tested, with all results being negative. Additionally, visitation was cancelled for today, but we are looking to resume visitation tomorrow. Please keep a look out for an email regarding visitation in the next few hours.

We are happy to report that we have tested all the staff and residents with all negative results.  Visitation will resume starting Thursday for everyone except those who currently are on Orchards.  Unfortunately, visitation cannot resume for Orchards residents until next Thursday, 8/12/21, due to the mandatory 14 day observation period.  You are able to continue virtual visitation via Facetime, Skype, Google Meet or Google Duo by emailing foothillacrescommunication@gmail.com .  Group activities, communal dining in the dining room and The Salon will also resume starting tomorrow.

Good morning,

Test results returned to us early this morning and confirmed a rapid test done on a resident in Sub acute is positive with COVID. This resident has been in isolation for the last 36 hours since the rapid was done as part of our outbreak testing, All precautions are being made to keep this incident to one person. Outbreak testing continues as all residents will be tested again in the next two days. We will keep everyone updated on when we will be resuming visitation.

As a follow up to the email sent yesterday, we unfortunately had a 2nd staff member test positive via a rapid test. As per protocol we also did a PCR test and will notify the families of the results which we hope to get tomorrow. That said, we are in full outbreak which means we will now need to have complete 2 rounds of outbreak testing to resume visitation which should be early next week.

Unfortunately, we have to suspend all visitation until at least Tuesday due to a positive test by a staff member.  All forms of visitation are affected including compassionate care, essential care, indoor visits, outdoor visits, DDD visitation, and even out on pass visits.  All communal dining and group activities have been suspended as well.  The Activity Programs will return to virtual activities through the tv system so all residents can still participate, and all meals will be served in the resident rooms.  The Salon will also have to be closed this week until testing is completed.  Anyone scheduled for this week will be carried over until
next week.
 
We have started outbreak testing and must have at least one round of negative testing completed for all staff and residents before we can resume any type of visitation.  We will keep you informed of any further developments via Constant Contact emails.  I will also send out any other information regarding visitation as needed.

I am happy to report the Hair Salon has officially opened. We were able to get 10 residents seen and as you could imagine, they were thrilled. It was so nice to see the Salon humming again. It may take a few more sessions but we are committed to getting to every resident as quickly as possible,

You know what I’m gonna talk about next….. the CALI scores. We were on a bit of a streak, staying in the LOW green designation for a little over two weeks. But we went back up to YELLOW moderate yesterday. The experts are pointing to the highly contagious delta variant. New cases more then doubled in the last two days, and its being reported that although vaccinated folks are doing well, the not yet vaccinated are susceptible. This means we educated our staff on being aware of where they travel to and whom they hang out with.

Speaking of vaccines, I was looking at our long-term care numbers and would like to share the following data:

If you look at the three long term units Magnolia, Maple, and Cedar, a whopping 98% of our residents (100 of 103) are fully vaccinated.

Additionally, we are now up to 85% our staff being fully vaccinated.

We are very happy to announce that our beauty salon, renamed The Salon at Foothill Acres, will be reopening starting Wednesday, July 7, 2021.  It has been a long time since some of the residents have had any type of salon services and we are certain they will be anxious to get a new hair style, but we are asking everyone to please be patient as we work our way through the facility and get to everyone.  In an effort to streamline this process, we have set up a convenient way for you request services by following this link to an online form.  This form is for setting up routine services, ex. Wash & Blow-Dry/Set/Style weekly, hair cut monthly and color every 6 weeks.  We have made easy to use pull down menus to select your choices and then how often you want that service completed.  There is also a selection if you do not wish to use the salon services.  This form will only have to be completed one time, after that, the services will be automatically done according to your selections.  If you ever need to change the frequency or want to request a different service, you can always submit a new request.  All requests must be in by noon on Friday for the following week to allow time for scheduling.

 

The Salon will also have an email address so you can communicate directly with the beautician and/or request services for special occasions when the resident may be going out to an event like a wedding.  The email is thesalonatfoohillacres@gmail.com.  Current plans are for the Salon to be open Wednesday-Friday.  The Salon and the staff at Foothill Acres will do everything possible to ensure your loved one is able to have an enjoyable experience while receiving their services.  However sometimes, due to physical limitations, it may be difficult to complete a wash and it may be safer and more pleasant for the resident, if their services coincide with a day their hair has been washed by the staff.  The Salon has been equipped with adaptive devices to assist with shampooing so the majority of the residents can enjoy the complete experience.

 

Please remember that all payments for salon services will be deducted directly from each resident’s Personal Needs Account (PNA) and funds must be in the account prior to any services.  If you have any questions regarding your resident’s account, you may reach out to Lori Kulik at boffice@foothillacres.com.  

 

Please take a minute to review the attached flyer for The Salon at Foothill Acres and I thank you in advance for your patience and understanding, as it may take a couple of weeks to get to every resident but we will be sure everyone has the opportunity to receive salon services if that is their wish.

As the nation as a whole and New Jersey specifically march toward higher vaccination rates, the infection rates for us continue to fall. The good news is based on the latest CALI score, Somerset County achieved a green LOW designation. This is very encouraging news. The one caveat I must share is the Northeast region which was green last week unfortunately went yellow MODERATE this week. So, it is my hope that we in Somerset County keep the low designation. Of course, we will be keeping a sharp eye on this as it’s the best indicator of how we are doing.

We are anticipating to reopening the hair salon for the residents in the coming days. Please look out for a separate email which will discuss this issue in detail.

I hope everyone is enjoying the beautiful weather we have been receiving. I know that the residents have been able to leave their rooms and get plenty of sunshine in our courtyard. Visitation is still going strong, and I would like to remind families that you can schedule 2x a week to visit your loved one. Wednesdays are our Outdoor Visitation day if the weather is cooperating. If we see temperatures above the resident’s preference; air quality and/or have rain in the forecast, all appointments made for Wednesday will be moved inside for the safety of our residents, or you may cancel your visit if you wish to not visit inside. 
 
We will be asking families if they are vaccinated and if we can have a copy of your vaccination card. You do not need to disclose if you have been vaccinated but we need to follow unvaccinated protocols if we do not have a copy of your vaccine card on file.
 
Families who are vaccinated and show us proof with a copy of their vaccination card can have a mask-less visit during our Outdoor Visits on Wednesdays if your loved one is fully vaccinated. To schedule your visit please go to http://calendly.com/foothillacres 
 
When you come to visit, we understand with the nice weather you want to take your family member to the garden or even just a stroll around the property, but with the current mandate given to us, we must ensure that the visitors remain distant from other residents. Part of the new guideline’s allow residents to go out on pass for non-medical trips out of the facility. Please read through the attached guidelines and If you are interested in taking part in this, please email foothillacrescommunication@gmail.com. 
 
In addition, it states in the new guidelines that we are able to transition back into group Activities. We are excited to bring everyone back together again. So far, we have had Live Entertainment in the Oak Room with our entertainer John Annadso to kick off the party, and our Penthouse Boutique took place in our upstairs Dining Room for residents to shop for anything from health and beauty, snacks, drinks and even puzzle books. We hope to have many more in the coming weeks as we get back to a sense of normalcy.
 
In order to encourage residents to come to our group Activities we will be scaling back our virtual video calls to 3x a week per resident between the times of 11:15 am to 7:00 pm with no calls between 2:00 pm and 4:00 pm so we can provide quality time for our group Activities and to the residents who are participating. 
 
Communal dining is all set and ready to start on June 1st starting with lunch. We are looking forward to having residents back in the dining room enjoying their meals with their friends filling the room with laughter. 
 
Our vaccination clinic for all residents and staff who are eligible to receive it is scheduled for June 4th at 9 am. We are offering the Johnson and Johnson vaccine to all residents who have not been vaccinated yet. This will make the percentage in Foothill of vaccinated persons even higher than it already is. I will update everyone with those numbers once we complete the 2nd round of shots. If you would like your loved one to be vaccinated, please email nursing@foothillacres.com by June 2nd.

I’m happy to report that once again this week Foothill Acres continued its trend of no new COVID cases. 
 
We also received the long-awaited revisions to the guidelines that govern visitation and indoor group activities/ communal dining. Our staff are currently going over the revisions and will be announcing to the families our plans on reopening the dining rooms for the residents in the very near future. As you can imagine, it does make a difference if a resident is vaccinated or not, but we are bound to make sure ALL residents are able to enjoy communal dining/group activities to the greatest extent possible. Once we have a plan, we will let you guys know.

I’m happy to report that this week Foothill Acres didn’t have any new COVID cases. Visitation has been in full swing indoors. As far as last week test results that we weren’t happy with, even though we have a very strong case to say the employee test was a false positive, including clearance from her personal physician that this was the case, we didn’t get clearance to count it as such so we are technically still in an outbreak. 
 
This week we had another successful vaccination clinic for all residents and staff who were eligible to receive it. This will make the percentage in Foothill of vaccinated persons even higher than it already is. I will update everyone with those numbers once we complete the 2nd round of shots.

Today we received word from the lab that a non-direct care Foothill employee that works in Magnolia tested positive, While we have every reason to believe this is a false positive, the Dept of Health is requiring us to consider it a true positive. There was no known exposure to residents or staff because this employee was always wearing appropriate PPE. However, this means effective immediately all visitations will be moved outside. We are conducting outbreak testing on all residents and staff. If we can determine this is indeed a false positive, I will update everyone via email.

I’m happy to report that with no new COVID cases at Foothill, we are steadily marching towards our goal of being COVID free for 28 days. That would allow us to request and “End of Outbreak” declaration from the local Health Dept which would enable Foothill to move forward on a few fronts. 
 
Visitation is in full swing, and I would like to give a shout out to Nancy Guerrera for spearheading the efforts of greeting, screening, and monitoring the visitation process. 
There have been a few requests for outdoor visitation even when we are having indoor visitation. In an effort to accommodate all reasonable requests, after discussing this with the Foothill team we decided to ask all families that prefer OUTDOOR visitation as their PRIMARY choice even when we are offering indoor visitation to reach out to us at info@foothillacres.com. Should we get enough interest, we will designate one day of the week for outdoor visitation. Please include your top 2 choices for which day of the week works for you. Obviously if there was a need to suspend indoor visitation, everyone will be switched to outdoors.

I’m happy to report that thankfully this week there were no new COVID cases at Foothill. The first round of outbreak testing has been completed on residents and staff. Due to the CALI score remaining at high, staff are continuing to be tested twice a week.

Dear Foothill Friends & Families,
 
Unfortunately, our lab notified us today that a direct care staff member has tested positive. This staff member is someone who not only wears the appropriate PPE all the time, but consistently wears an N95 mask. All residents were negative during this round of testing. We will start testing all staff and residents again on Friday (4/2). 
 
All scheduled 25 minute indoor visits will remain outside (weather permitting) until the next round of testing is complete with negative results. Please continue to schedule these visits by going to www.Calendly.com/foothillacres. It is labeled as “indoor visits” but we will automatically change the location, depending on the testing circumstances, without anyone having to re-book with us.  
 
The weather forecast for tomorrow does not appear to be promising for outdoor visitation due to the extreme cold temperatures for this time of year. It is a very hard decision to make, but we feel it is best to cancel the scheduled visits for tomorrow. There is warmer weather forecast for the beginning of next week and without any rain, which should make visiting enjoyable. Mondays have morning and afternoon times available as it doesn’t interfere with the Activity programs. Please remember each resident can receive 1 visit a week with no more than 2 visitors per visit. We encourage families to coordinate with each other when scheduling to avoid conflicts.

Hello everyone, 
 
Unfortunately, we were notified by the lab that two employees (direct and non-direct) have tested positive for COVID-19. Both employees are asymptomatic and have been asked to quarantine. No potential exposures were identified at this time since both employees wore the appropriate PPE while in the facility. We have been testing employees twice a week in accordance with the current CALI score and will continue to do this in the coming week. All residents will also receive outbreak testing starting Monday. 
 
ALL indoor visitation is cancelled until the first round of outbreak testing is complete, however; we have made arrangements to continue with outdoor visitation on the current schedule. Compassionate care visits and essential care visits will continue as scheduled as long as testing is maintained.

Dear Foothill Families & Friends,

 

Good News!!  We have received a new memo from the NJDOH that has some new guidelines regarding indoor visitation.   In the previous directive, one of the things that limited indoor visitation was the CALI score in the community.  This score went up last week and would have affected us, however under the new guidance, and since over 70% of the our residents are vaccinated (we are at 90%), we are now will be able to resume indoor visitation by appointment this week, starting on Friday!!! 

 

The new guidelines allow indoor visitation for all residents (regardless of vaccination status or facility’s phases) except now we have different steps that must occur in an outbreak in order for indoor visitation to be continued.  Under the new guidelines, indoor visitation would be limited under the following circumstances:

 

  • Indoor visitation for UNVACCINATED or NOT FULLY VACCINATED residents should be limited if the facility is in a region with a CALI score that is high or very high AND less than 70 percent (<70%) of the residents in the facility are fully vaccinated.
  • Indoor visitation should be limited for residents with confirmed COVID-19 infection, whether vaccinated or unvaccinated until they have met the criteria to discontinue Transmission Based Precautions.
  • Indoor visitation should be limited for residents under quarantine, whether vaccinated or unvaccinated, until they have met criteria for release from quarantine. This means that these residents will not be eligible for a visit until their 14 day quarantine has concluded.

 

When a new case of COVID-19 among residents or staff is identified, we have to immediately begin outbreak testing and suspend all visitation until at least ONE round of facility-wide testing is completed.  We will then be able to resume indoor visitation as long as there is evidence that the transmission of COVID-19 is contained to a single area of the facility.   So if the first round of outbreak testing reveals no additional COVID-19 cases in other areas, then indoor visitation can resume for residents in areas with no COVID-19 cases. However, we are required to suspend indoor visitation for the affected areas until the we meet the criteria to discontinue outbreak testing.  If any additional areas are affected, then we would have to continue to suspend all visitation for all residents until we meet the criteria to discontinue outbreak testing (no new cases identified for 14 days since last positive case).

 

For the safety of all of the residents and staff of Foothill Acres, we have to limit the number of visitors per resident, the total number of visitors in the building at any given time, and the movement of the visitors while in the building.  In order to ensure all residents are able to receive visitors on a weekly basis, visits are still limited to 2 visitors (ages 14 or older) per visit and one visit per week, per resident. Family members will need to coordinate with each other when scheduling visits.  Only one form of visitation may be scheduled per day so, on days that indoor visitation is scheduled, all other forms of visitation (including video calls) will be canceled.

 

Fully vaccinated residents can choose to have close contact (including touch) with a visitor as long as the person and resident are wearing a tight-fitting masks (i.e. Surgical mask).  We will be encouraging the use of hand sanitizer before and after all personal contact.  Please try to maintain social distancing and limit contact with any other residents and staff.  All other infection prevention protocols must be maintained during indoor visitation.

 

Indoor visits will be for 25 minutes and appointments can be scheduled throughcalendly.com/foothillacres . Earlier today we were notified that a non direct care employee tested positive. This employee last worked in the facility on Friday, was not feeling well on Saturday, and got tested outside the facility on Sunday. I’m happy to report that thankfully this employee took her personal responsibility seriously and was extremely careful not to be without her PPE at all, and chose to eat lunch while practicing social distancing to further increase the chance of not getting or giving the virus from or to anyone else. Those choices protected her fellow employees from potential exposure. To be safe, all employees who worked with this employee for the 48 hours prior were rapid tested today and all results were negative. We will be keeping a close eye on those employees for the next several days. Unfortunately, this does reset the clock for the outbreak, and we are now calculating our day14 for EG visitation to be Sunday March 28. 
 
CALI scores remain steady for the last 3 weeks at moderate, which is the silver lining here since that is a good indication of the overall health of the county.

I’m happy to report that thankfully this week there were no new COVID cases at Foothill. Residents and staff are continuing to be tested weekly. We will continue outbreak testing facility wide until next Thursday, March 18. 
 
With the weather becoming milder, we know visitation is on everyone’s minds. Additionally, the CDC just released new guidelines on visitation. Foothill Acres is waiting for NJ Dept of Health to issue their guidelines which usually mirror those from the CDC. Once we have them in hand, we will have a much better idea as to when we can open up for indoor visitation and under what circumstances. For now, CALI scores still play a major role in determining visitation status.

In the meantime, all residents of that unit were rapid tested today with all tests coming back negative. Staff are continuing to be tested weekly. We will continue outbreak testing facility wide. 
 
ALL visitation is still stopped, with the only exceptions made for those End-Of Life situations where a doctor deems their patient actively dying. This does reset the clock for general visitation as well.

Last evening, we were notified that one of our residents in Cedar Unit (which is being used as overflow for Sub Acute patients) tested positive. This resident was inside the 14-day isolation period from admission from the hospital. We are continuing outbreak testing facility wide. All staff and residents were tested today.
 
Additionally, all of the residents on Cedar were rapid tested with all results returning negative. We will continue to keep a close eye on all staff and residents for the next two weeks. 
 
ALL visitation is still stopped, with the only exceptions made for those End-Of Life situations where a doctor deems their patient actively dying. This does reset the clock for general visitation as well.

Hello everyone, 
 
Late last night we were notified that one of our residents in Magnolia Unit tested positive. This is the first resident positive we’ve had in approx. 8 weeks. We are now required to resume outbreak testing facility wide. All staff were rapid tested today, and so far, the results have all been negative. All residents will be PCR tested tomorrow. Unfortunately, even one resident testing positive is considered an outbreak, which means ALL visitation is immediately stopped, with the only exceptions made for those End-Of Life situations where a doctor deems their patient actively dying. This does reset the clock for general visitation as well. 
We hope this will remain an isolated incident. We are continuing to screen all employees at the main entrance for every shift prior to gaining access to the floors. We are continuing education to our staff, reviewing the importance of handwashing and wearing appropriate PPE.

Hello everyone,
 
Today we were notified that one of our non-direct care employees tested positive. This is the first positive we’ve had in approx. 7 weeks. We hope this will remain an isolated incident. The employee was in the facility yesterday but was wearing appropriate PPE and worked on a unit that nearly all the residents were vaccinated. At this point, we are waiting for guidance from the local Health Department if this is considered an outbreak. If it is, ALL visitation will need to be put on hold until we get thru outbreak testing. AT THIS TIME, WE ARE STILL ABLE TO HAVE ESSENTIAL CAREGIVER AND COMPASSIONATE CARE VISITATION.
 
Now for the good news. The Somerset County CALI score dropped from HIGH (orange) to MODERATE (Yellow) What does this mean for us? Well, to protect the long-term care population, once the local CALI score drops a level, we are required to be two weeks behind the community. Simply put, once the Somerset County CALI score is at MODERATE (yellow) for two full weeks, we can than potentially restart indoor visitation.

Dear Family and Friends,

We are happy to announce that our vaccination clinic with CVS Pharmacywas a HUGE success. 94% of our eligible residents received the vaccine, and 78% of our eligible staff. Our overall population at Foothill is 86% vaccinated. We are grateful to the wonderful Foothill Acres staff that worked diligently with CVS Pharmacy to make this happen. We are looking forward to brighter days ahead, and appreciate your continued support.

I’m happy to share that we continue to have no new positives for residents or staff this week. That puts us at 30 days clean. We applied for and received the official “End of Outbreak” declaration from the Dept of Health. Visitation is still not available due to the high CALI score.

In other news, the 2nd vaccine clinic with CVS was a huge success. A whopping 94% of our eligible residents received the vaccine, plus 78% of staff got the vaccine as well. This puts the overall Foothill population at 86%, well over the 70% needed for herd immunity. I am very grateful to the wonderful Foothill staff that worked together with the CVS team to get this job done. Special shout out to our Director of Nursing Diana, and the dynamic duo Nancy and Mary.

I’m happy to share that we haven’t had any new positives for residents or staff this week. That puts us at 23 days clean. There is still a long way to go before we would be able to resume visitation for the general population but this is definitely moving us in the right direction. We are still bound to all other factors that govern the allowance of indoor visitation. Right now, assuming we reach 28 clean days, AND we get an official “End of Outbreak” declaration from the Dept of Health, we still need to consider the CALI score. I will reiterate what was shared with the Foothill families about 2 months ago. 
 
As per Executive Directive 20-0261 indoor visitation is contingent on several factors including facility outbreak status and CALI scores. COVID-19 Activity Level Report or CALI scores are released weekly by the NJDOH and are based on positivity rates in the State. The latest CALI score shows the whole state as HIGH, and quoting from Executive Order 20-0261

I’m happy to report this past Wednesday we reached a milestone of 14 days without a new positive. This allowed us to open limited visitation for our Essential Caregivers. 
 
We are preparing for the 2nd vaccine clinic with CVS which is scheduled for a week from this Monday and Tuesday, Jan 25th and 26th. 
 
We are continuing weekly test of residents and staff. We now have our sights set on reaching 28 days without a new COVID positive. Should we be successful, we would be one step closer to ending the current outbreak in the facility which would enable us to open up visitation for the rest of the residents. I will keep everyone updated on our progress as I understand many families have been unable to visit for quite some time. We would love to see residents and families spend time together and we are working diligently to make that happen. I am unable to predict what day we would possibly open for regular visitation since there are many things outside of our control at play here. I appreciate your understanding.

Hello All, 
 
I hope everyone enjoyed the Holidays. 
 
I’m happy to report this past Monday and Tuesday we had a very successful vaccination clinic. 
 
Of the residents that were eligible to receive the vaccine, a whopping 93% got the first dose. Additionally, 71% of staff received the vaccine as well. That gives Foothill an average of 82% of the people in the facility chose to be vaccinated. We anticipate this number to climb a bit more at the next clinic, as there may be some residents and staff that would opt to get their first dose then. We also gave 14 Essential Caregivers the vaccine. 
 
I want to thank ALL our families and staff who did an incredible job at getting everything ready for the clinics as this avoiding potential issues. I also want to publicly thank CVS for the excellent personnel they sent to Foothill to administer the vaccine. They were friendly, professional, and efficient. We look forward to the 2nd clinic with CVS. 
 
We are continuing to test residents and staff. We have not gotten all our lab results back yet so the jury is still out on results. That said, IF there are no new positives, visitation for Essential Caregivers will be Wednesday, January 13th

Two days ago, we tested all the residents to establish a baseline. Unfortunately, one came back positive from Maple. That resident was not having any symptoms but was moved to the COVID wing. These are our current numbers:

I’d like to share some great news with the extended Foothill Acres family. We have been scheduled to offer the COVID vaccine to our Residents, Staff, and Essential Caregivers this Monday, January 4th, 2021 and Tuesday January 5, 2021.  
 
Like previously discussed, CVS Pharmacy will be handling the entire process. I am not yet aware of which vaccine they will be offering but I have heard from other nursing homes it is the one from Pfizer.
 
I’m also happy to see a lot of families have sent in their consent forms. If you have not already done so, PLEASE send it in as soon as possible so we will be ahead of the curve. I anticipate Monday to be a very busy day for all of us, and am asking for your help in being prepared as best as we can be.

As we head into the winter months, we are getting ready to offer the vaccine for all residents who wish to take it. We are strongly recommending that every resident take the vaccine. Ultimately, this is a personal choice, and the facility will honor any decision that a resident and/or healthcare proxy makes. 
 
We have been in contact with CVS who will be overseeing the vaccine program. As of today, we have heard that the vaccine should be available to nursing homes not before Dec 28. If the resident is alert and oriented and generally completes their own paperwork, we will have them (with the help of Foothill staff if requested) complete the forms. If you are the Responsible Party for a resident in the facility, please complete the attached consent form and return it to the facility at safety@foothillacres.com. We would like to be prepared ahead of time so when the green light is given, we are ready to go. 
 
We are continuing to test staff weekly. We have been notified by the lab that 3 more staff members and one resident from Maple have tested positive. That resident was moved to the COVID wing.

Today the facility continued to test residents and staff in an effort to identify any additional COVID positive individuals. We were puzzled by the results, because two residents in the Maple unit that were positive who were not displaying any symptoms and have a medical history which give us strong reason to believe they were false positives. Infectious Disease consultants ordered another rapid which came back negative. PCR tests were also sent to the lab and we are waiting for results which we anticipate to get back by Thursday the latest. Those residents were immediately moved to the COVID unit for safety. Additionally, the entire Maple unit was rapid tested with all results coming back negative.

Over the weekend, testing revealed 1 Sub Acute resident and 1 staff member with a positive COVID PCR test result. This resident is asymptomatic at this time and has been moved to the covid unit. The staff member was wearing full PPE. The entire Sub Acute unit remains under isolation protocols until the 14 day period of no new covid positive cases. All staff on this unit will be wearing full PPE at all times.

Another employee tested positive to the rapid test yesterday. This employee was not in facility on Wednesday and just came in to be tested on Thursday. This employee is not a direct caregiver, and was wearing appropriate PPE during previous work schedule. That brings our totals active COVID to:

On another note, there has been a huge focus on the vaccine in terms of when it will be available to facility residents and employees. I’m grateful that the NJDOH has deemed residents in Long Term Care facilities like Foothill Acres and the healthcare workers that care for them the highest priority for roll-out. Foothill Acres has partnered with CVS Pharmacy who will be handling the entire vaccination program. We expect to be contacted by CVS early next week with more detailed information. Once the information becomes available I will forward it to residents and their families. In the meantime, please know that we continue to follow state guidelines at every turn. Keeping residents and staff safe is not the highest priority, it’s the only priority.

Testing is continuing at Foothill Acres. Last night we were notified that 4 residents in the Sub Acute unit tested positive. There have no new staff positives since last week, so we are considering this a continuation of last weeks outbreak. Our current numbers including last week are:

From the 7 residents, two will be considered  recovered tomorrow and will be removed from the isolation unit.

I’m happy to report that outbreak testing was completed and there were no additional positives. That means we currently have zero residents and zero employees with COVID. This means that visitation for Essential Caregivers, Compassionate Care and Hospice only is now possible. Our wonderful team has already reached out to all families that fall under the permitted categories. 
 
IF YOU ARE ONE OF THOSE THAT ARE ABLE TO ENTER THE FACILITY, PLEASE MAKE SURE YOUR TESTING IS UP TO DATE. WE WILL NOT BE ABLE TO ACCOMMODATE ANYONE WHO HAS MISSED EVEN ONE TEST.

Outbreak testing at the facility is still not wrapped up due to longer wait times for results from the labs. We expect to get results from the last round of testing in the next day or so. Currently, results have been encouraging and we are hopeful of future outcomes that will allow us to emerge from the outbreak status. As far as resuming indoor visitation, the facility is still following the guidance released by the State. As per Executive Directive 20-0261 indoor visitation is contingent on several factors including facility outbreak status and CALI scores. Since we are still in outbreak testing until we get all test results back, visitation is still not possible. The other thing we must consider is the COVID-19 Activity Level Report of CALI score. CALI scores are released weekly by the NJDOH and are based on positivity rates in the State. You can find them at https://www.nj.gov/health/cd/statistics/covid/index.shtml
 
The latest CALI score was upgraded to HIGH, and quoting from Executive Order 20-0261
“CALI score High or Very High = Visitation should only occur for compassionate care, end-of-life and essential caregiver situations according to the core principles of COVID-19 infection prevention, NJDOH guidance and directives”. This factor restricts the facility from allowing indoor visitation except for compassionate care, end-of-life and essential caregiver situations, even when we graduate from the outbreak status, and even those visits “should not be routine.” Virtual visitation can still be scheduled by emailing foothillacrescommunication@gmail.com.
 
We look forward to a time when we will be able to throw open our doors to everyone but under the current circumstances it just isn’t possible.
As the holiday season is fast approaching, we’ve put together a flyer (attached) which outlines our policy for this year. As with everything else, NJDOH has given us directives which govern our policy. DOH strongly discourages families from taking their loved ones out of facility for the holidays, even for a short time, but recognizes the right of residents and their families to make those decisions. Please be advised, anyone who does leave the facility will be required to quarantine in the facility for 14 days upon their return. Please see the attached flyer for further details.

Good afternoon,

 

Last night Foothill staff received positive COVID test results from four residents in the Sub Acute unit. With this new outbreak Foothill is following protocols to isolate the positive residents in the designated separate hallway in the sub-acute unit behind double doors with each resident under quarantine in their own room. All residents and staff will be tested twice in the next 2-5 days with additional testing as needed. Residents are being tested today, staff tomorrow. Once round one is complete, we will initiate round two. At this time, all staff are negative from the standard weekly testing. All staff that were identified as prolonged contact with these residents have been removed for the schedule as well.

This new development will continue the need to keep all visitation suspended indefinitely. If you had a visit scheduled for Nov 19 or later, please do not come to the facility. You do not need to go to the website to cancel.  We will send out a notification when it is safe to resume visitation. all virtual visitation modules are still available.

Hello again, 
Late last evening Foothill Acres received a positive test result from a staff member as part of the weekly testing we are conducting of all staff. This employee displayed no symptoms (asymptomatic) when they were screened at the beginning of their shift. This staff member worked on Maple Unit. Under the guidance of our physicians, we have been advised that we need to quarantine and test all residents that were on this staff members assignment, and as well any staff who had extended contact with this employee for 14 days. We will be closely monitoring all residents on the unit.
 
 As in the previous case, due to the current circumstances, we need to temporarily suspend ALL visitation, with exceptions only for residents that are actively passing. We know how frustrating this is to everyone considering we just started indoor visitation yesterday. Unfortunately, we are left with no choice to do this for the safety of the residents and their families.

I am happy to report that we have completed our required 14 day COVID free period. This is significant because under the latest CDC guidelines released on October 20th, we now are able to offer limited indoor visitation. We are planning on having the first indoor visitation on Wednesday, November 4th. The guidelines for indoor visitation will be emailed to all early next week. It will outline everything you need to know on how to set up an appointment, location of the visits, and a general overview of the process. As always, we are available to answer any and all questions you may have.
 
Two things I need to point out are:
 
1. When the indoor visitation starts, it will replace the outdoor visitation program.
 
2. Everyone will be required to use the same online appointment scheduler to schedule an appointment to enter the facility. Please don’t show up if you don’t have a confirmed appointment and put us in the uncomfortable position of refusing entry.

As a follow-up to the previous letters, I’m happy to report that all tests results from residents and staff this past week returned negative for COVID, thus successfully establishing a new baseline for all. This is the result we were hoping for and allows us to restart all visitation effective this Monday October 26. Please use the online scheduler which can be found at www.calendly.com/foothillacres The website is active and ready to take your appointments. Essential Caregiver, DDD, and Compassionate Care visits are also resuming on Monday.
 
We are working diligently to have indoor visitation. There are many guidelines that need to be met, one of which is to be COVID free for 28 days, and then to get a letter from the local Health Dept attesting to the end of the outbreak. Once we meet all criteria for indoor visitation, you guys will be the first to know.

Hello again,
 
Late yesterday evening Foothill Acres received another positive test result from another staff member. As was the case yesterday, this employee displayed no symptoms (asymptomatic) when they were screened at the beginning of their shift.  This staff member worked on all long-term units. Under the guidance of the local Health Department they advised us that we need to establish a new baseline for all residents and employees. We will be testing all residents starting Saturday and then a 2nd time on Monday. All staff will be tested no later than Monday and again no later than Wednesday. Once we establish the new baseline we will test as needed. Families are within their rights to refuse testing. If anyone does NOT want to have their family member tested, please reach out to the facility right away to let us know.
 
The local Health Department further advised us that due to the current circumstances, we need to temporarily suspend all visitation, with exceptions only for residents that are actively passing. We know how valuable the Essential Caregiver, DDD, and Compassionate Care visits are, but we are left with no choice to do this for the safety of the residents and their families.

I hope everyone is doing well.
During standard testing of all residents admitting to the facility, we had a resident who returned after a brief stay to the facility and had a positive COVID test result. As per protocol, upon readmission the resident was put into 14 day isolation. The resident was retested both with a PCR and rapid, and both came back negative. 
While we have every reason to believe that this is a false positive, we are required by NJDOH to notify residents, families and staff of all positive COVID test results.  Once we are able to determine definitively, I will reach out again.
On another note, we did have an employee who worked on the Sub Acute unit test positive. This diagnosis was confirmed by a rapid test. The employee was screened at the beginning of the shift and asymptomatic and was cleared for work.
While it is concerning to have a positive result, I want to say that this is exactly the reason why we test. It allows the facility to protect the residents and staff by knowing early who is infected and who needs to stay out of the facility. Without testing, this employee could have reported for work many more times thus increasing exponentially the risk to everyone. By having the testing done weekly, we were able to limit it to only one shift.

Background & Lessons Learned:

Foothill Acres Rehabilitation &amp; Nursing Center, as many other nursing centers, has experienced the heartbreaking impact of the COVID-19 pandemic. Living in a healthcare setting brings inherent risks due to the vulnerability of the residents we care for and the communal lifestyle. Our efforts to provide a homelike environment that promotes independence and person directed care, can contribute to the spread of a respiratory infection such as COVID-19 when aggressive restrictions are not implemented early in the outbreak. Finding a balance between the need to be aggressive and honoring the basic rights of our residents and families will require a comprehensive plan that includes methods to communicate with residents, visitors and the staff so we can all work together to limit the exposure and to prevent the spread of respiratory infections including COVID-19.

POLICY:

Foothill Acres Rehabilitation &amp; Nursing Center has developed a comprehensive response plan to address the global pandemic due to COVID-19. The plan will be in place to protect the residents, staff and visitors from respiratory infections including COVID-19.

PROCEDURE:

  •  The facility will create a COVID-19 Committee team consisting of the following:

  • The facility’s COVID 19 Response Coordinator will be: Pinny Goldblatt, Administrator.
  • The facility will coordinate with the transport company if a presumptive or confirmed COVID-19 is going to be transported. Face mask will be provided to the resident for transport to protect the transport staff and receiving facility staff.
  • The facility physician will coordinate with the receiving facility’s physician to ensure report is accurately communicated and precaution is in place.
  • Dedicate equipment in isolation rooms. If not possible, clean and disinfect equipment using the EPA-registered disinfectant before use with another resident within the cohort.
  • Conduct routine cleaning and disinfection of high touch surfaces and shared medical equipment using EPA-registered disinfectant.
  • Increase frequency of routine cleaning and disinfection.
  • Prioritize rounding in a “well to ill’ flow to minimize cross-contamination.

Communication with Residents and Families:

POLICY:

It is the policy of this facility to inform residents, their representatives, and families of the residents by 5pm the next calendar day following the subsequent occurrence of either: each time a single confirmed infection of COVID-19 is identified or whenever three or more residents or staff with new-onset of respiratory symptoms occurs within 72 hours of each other.

PROCEDURE:

  1. Nursing Administration will notify the Administrative Staff if the facility meets either of the criteria listed above.  The Administrative Staff will gather all required information as directed in CMS rule: 42 CFR 483.80(g) and with CMS guidance at https://www.cms.gov/files/documents/gso-20-29-nh.pdf.
  2. Administrative Staff will send required information out via email and text to all residents, their representatives, and families of the residents by 5pm the next calendar day following the subsequent occurrence of either: each time a single confirmed infection of COVID-19 is identified or whenever three or more residents or staff with new-onset of respiratory symptoms occurs within 72 hours of each other.
  3. Residents will be notified of a new outbreak via our closed circuit television system which will be posted by 5pm the next calendar day following the subsequent occurrence of either: each time a single confirmed infection of COVID-19 is identified or whenever three or more residents or staff with new-onset of respiratory symptoms occurs within 72 hours of each other.
  4. The required information along with an emergency number for urgent calls and complaints will be posted on our website by 5pm the next calendar day following the subsequent occurrence of either: each time a single confirmed infection of COVID-19 is identified or whenever three or more residents or staff with new-onset of respiratory symptoms occurs within 72 hours of each other.
  5. Administrative staff will continue to send out information to all residents, their representatives, families and employees at least once weekly for any facility updates.
  6. Foothill Acres will also be providing weekly conferences on a scheduled basis for all who like to attend. Scheduled calls will be posted on facility’s website and/or social media along with e-mail address will be made available to all residents, their representatives, families and employees for any urgent issues or concerns that may need immediate attention.

General Facility Control Measures

  • Establish testing capacity to identify SARS-CoV2 through Rapid Antigen testing or through ACCUREFERENCE Laboratory and ACULABS Laboratory to conduct test, and provide appropriate specimen collection materials.
  • Ensure sufficient supply of Hand Hygiene resources in the facility.
  • Evaluate personal protective equipment (PPE) and report level to corresponding agency/OEM/LDOH/NJDOH.
  • Educate staff on infection prevention practices, control measures for COVID-19.
  • Facility will implement Universal Source control by providing facemask to person in the facility. As an alternative, cloth mask will be provided in the event of supply shortage for staff that does not provide direct care to the residents.

Daily Reporting

  • Complete line listing for residents / staff.
  • Submit line listing to Hillsborough Township Health Department per instruction. Hillsborough Township Health Department representative Siohban Spano.

Admission, Transfer and Readmission

  • Close the unit affected to new admissions except as needed to cohort individuals and staff.
  • Consider closing to new admissions if the facility is unable to appropriately cohort. This does not include readmission back to the facility.
  • When transferring a resident to another facility / hospital, transporting company will be informed of the Transmission Based Precaution in place as well the as receiving facility.
  • New admissions and readmissions will be monitored for signs of COVID-19 They will be placed in private rooms (if available). If feasible, a cohort will be created for this population.

Emergency Staffing Strategies:

POLICY:

In the event of an emergency, the Administrator or DON will make the decision to utilize emergency staffing strategies as necessary to provide for care and treatment of residents.

PROCEDURE:

  1. All employees in the facility will be notified of the decision to utilize emergency staffing strategies.
  2. In the event the facility has difficulty with staffing the facility in an event of an emergency, the following will take place:
    a. Remove tasks from the nursing department that does not need to be completed by a CNA or Nurse including but not limited to passing out water, answering call bells, passing out snacks and meal trays to alternate employees.
    b. Restorative nursing staff will assist on the units as well as Rehab staff within their scope of practice
    c. Nursing Administration (DON, ADON, Unit Manager and Supervisors) may need to work on the units as needed
  3. Dietary department may utilize paper products in order to free staff to assist in other areas.

Virtual Communication:

POLICY:

To continue to provide the vital opportunity for family members to interact with their loved ones while minimizing the chance of the spread of COVID-19.

PROCEDURE:

  • Scheduling for all video chats will be done by the Activity Department.
    o All family members were notified to send an email for all virtual visitation requests to
    foothillacrescommunication@gmail.com at least 24 hours prior to requested video chat time.
    o Email addresses for new admissions are obtained by Admissions and/or Receptionist.
    o Chats are scheduled daily from 10:00 am to 7:00 pm and are limited to 10 minutes.
    o A daily schedule will be printed and distributed to each unit and Activity Supervisor so everyone knows what chats are scheduled for the day.
    o Activity Director and/or Activity Supervisors will coordinate who will be assigned to carry out each video chat scheduled.
      – On units with suspected or confirmed COVID-19 or 14-day quarantine, the Activity Supervisor or Director will coordinate with Unit Managers to identify any residents that could done by Nursing staff while in the rooms.
    o When video session is complete, staff are to log the results and notify Activity Director or Supervisor of any issues during the call.
  • For units/rooms with no confirmed or suspected cases of COVID-19 or residents on 14-day quarantine:
    o Activity staff or other facility staff will complete video chats with residents.
    o All tablets and iPads will be wiped down with disinfecting wipes before and after each use.
    o If resident has difficulty hearing, there are headphones with a mic available for that resident to use and keep at their bedside. Headphones will be labeled with the resident’s name on them.
  • For units/rooms with at least 1 confirmed or suspected case of COVID-19 or residents on 14-day quarantine:
    o All tablets and iPads will be wiped down with disinfecting wipes before and after each use.
    o All tablets and iPads will be placed in a gallon zip lock bag prior to entering the resident room.
    o Staff will don all appropriate PPE prior to entering room.
    o Stands for devices are available for use to promote social distancing once virtual call has been established. Stands must be wiped down before use.
    o If resident has difficulty hearing, there are headphones with a mic available for that resident to use and keep at their bedside which will be labeled with the resident’s name on it.
    o Dispose of zip lock bag when video chat is complete and wipe down tablet and stand with disinfecting wipe.
    o Activity staff will complete video chats for residents who are not confirmed with COVID-19 or showing any symptoms. To minimize the traffic in an isolation room, the CNA or other nursing staff may complete video chats for those who are suspected or confirmed with COVID-19 or currently on 14-day quarantine.

Infection Prevention and Control

  • In the event of a COVID 19 outbreak occurs, the facility will encourage all visitors and non-essential healthcare personnel to restrict or limit their visitations, except in certain compassionate-care situation.
  • Evaluate all persons who enter the facility for signs/symptoms of COVID-19. Temperature checks at entry point. Temperature checks at the start of each shift for all employees.
  • The facility will monitor any visitors/vendors/physician for temperature and any signs of respiratory symptoms. The following will also be assessed: history of international travel with 14 days or travel to any of the high-risk areas 14 days, history of exposure to a presumptive or positive COVID-19. 
  • Visitors with respiratory symptoms will not be permitted to enter the facility at any time.
  • Facility will require visitors who are cleared to enter the facility to perform hand hygiene and use of facemask and restrict visitation to resident’s room or private areas.
  • Signage for visitation, cough etiquette and hand hygiene will be posted in several locations within the facility.
  • Visitation information will be communicated through mass letter, telephone calls and posting of signage.
  • Active screening for residents for fever (Temperature of 100.4) and other COVID-19 symptoms every shift.
  • Any supply delivery will be dropped off by the loading dock and/or right by the reception area.
  • Alcohol-based hand sanitizers for hand hygiene will be provided in the hallways and on medication carts based on supply availability.
  • PPE needed will be made available outside the doors for easy access to the staff.
  • Facility will provide EPA disinfectants to allow for frequent cleaning of high touched surfaces and shared resident equipment.
  • Suspend communal dining and all group activities as needed.
  • Residents are encouraged to remain in their rooms. If the resident leaves their room, they should wear a face covering or face mask; perform hand hygiene, limit movement in the facility and perform social distancing.
  • Waste receptable will be provided for PPE 
  • Implement Standard and Transmission-Based Precaution including use of N95 mask or other respirators.
  • Signs posted for direction of PPE to use in residents on Transmission Based Precaution

Resident Management / Cohorting

  • Implement cohorting plan that allows for separation of residents, dedicating staff and medical equipment to each of these cohort and allowing necessary space to do so at the onset of an outbreak.
  • In the event staffing cannot be designated specifically in the COVID-19 unit, Staff will work from clean unit to COVID Unit., Staff will cluster work load from clean resident to COVID residents.
  • Staff will don COVID-19 recommended PPE:  N-95 or respirator, eye shield, gloves and gown when entering resident rooms.
  • Laboratory confirmed COVID-19 positive residents will be relocated to designated cohort. Private rooms will be provided if feasible. Cohort with another COVID-19 resident maybe done when private room is not possible.
  • Person Under investigation will not be moved to COVID-19 cohort until results are confirmed.
  • Roommates of COVID-19 residents are considered exposed and should not share a room with other residents unless they have tested negative and has been asymptomatic for 14 days from last exposure.
  • Facility will adhere to the CDC guidelines for the discontinuation of Transmission-Based Precaution.
    Symptom-based strategy
  •   At least 3 days (72 hours) have passed since recovery defined as resolution of fever without the use of fever-reducing medications and improvement in respiratory symptoms (e.g., cough, shortness of breath); and,
  • At least 10 days have passed since symptoms first appeared or
  • At least 7 days have passed with a negative test result and asymptomatic or improvement of symptoms.

Staff Management

  • All residents will be provided mask and instructed to cover their nose and mouth when around others, as tolerated.
  • Enforce use of universal facemask (source control) for staff while in the facility in addition to active screening of symptomatic staff.
  • Staff will be screened by getting their temperature taken and completing a screening tool at the start of their shift
  • Any staffs that show symptoms consistent with COVID-19 will be asked to put on a mask and leave the facility. Such staff would require clearance from IP before returning to work.
  • Identify staff who maybe at higher risk for severe COVID-19 and attempt to assign to unaffected units.
  • Educate and reinforce sick leave policy including not to report to work when sick.
  • Assess staff competency on infection prevention and control measure including putting on and taking off PPE.
  • Conserve use of PPE using CDC guidance for PPE contingency strategies.
  • Consider staffing contingency for shortage (refer to separate policy).
